Avrupa İnsan Hakları Mahkemesi'ne yapılacak başvurular, Mahkeme tarafından belirlenmiş bir formun doldurulması ile gerçekleştirilir. Bu form dışında bir dilekçe ile başvurmanız durumunda AİHM tarafından size resmi başvuru formu iletilecek ve başvurunuzun bu forma belirtilen formata uygun olarak yapılması istenecektir. Zaman kaybına neden olabilecek bu durumla karşılaşmamak için başvurularınızı bu formu kullanarak yapmanız gerekmektedir.
